Puppy Contract and Health Guarantee  (Limited)
 

Initial Health Exam: BUYER has 3 days from the time puppy leaves the custody of the BREEDER to have the puppy examined by a veterinarian of his/her choice at BUYERS expense to verify the general health of the puppy. The puppy is sold with a one-year limited warranty that is free of genetic life-threatening physical defects.  The BUYER may return this puppy to the BREEDER at the BUYER’S expense to receive a full refund if this puppy is found to have a life-threatening illness. This does not include minor illness requiring short-term medication. Proof of examination and diagnosis must be provided to the BREEDER within the three day period in order to receive any reimbursement of funds. The BREEDER reserves the right to a second veterinary opinion.
The BREEDER should be contacted immediately with the name and number of the vet responsible for performing the exam. The vet will also need to provide IN WRITING all of the results and a diagnosis. The BREEDER is not liable for any accident or injury of the animal once it leaves the Breeders premises. This warranty does not cover viral illnesses, parasites, infections, improper bites, hernias, Giardia and Coccidiosis. It also does not cover illness due to the ingestion of foreign objects, food, chemicals, stress-related issues or physical injury.

The BREEDER states that this puppy has received at least one shot of the following vaccinations – Distemper, Adenovirus Type 2, Parainfluenza, and Parvovirus.  The BREEDER will provide the BUYER with the puppies vaccine and worming records at the time of pick up

The BUYER shall keep the dog as a household pet.
The BUYER shall provide the dog with:

Adequate food, fresh water, shelter, affection, and medical care upon sickness, disease, or injury. Maintenance should include an annual health exam and routine vaccinations as recommended by a board-certified veterinarian. A fully fenced yard or other humane means of exercise. The dog shall not be kept chained or constantly outdoors. A collar and identification tag and/or microchip. Never permit the dog to roam unattended.

The dog shall reside with the BUYER.
The BUYER shall notify the BREEDER immediately if the dog cannot be kept by the BUYER.
The dog shall not be sold, transferred to any other or this contract is null and void.
The BUYER and BREEDER agree to keep each other informed of their current addresses.
The BUYER agrees that it is the BUYER'S responsibility to accept the dog as described. The BREEDER has provided and the Buyer has acknowledged a full disclosure of the health, habits, and temperament of the dog.
